Required Health Examinations and Immunizations

Health Requirements Compliance Deadline: Students must have all required health information on file with the school office by Oct. 15,2015.

Lead Screening

Required for all Pre-K and Kindergarten students


Immunizations are required by the Illinois School Code and part 665 of the Child Health Examination Code. Please note there are new immunization requirements for the 2015-2016 school year. Please contact your health care professional for more information on the requirements for your student.

Health Examination

All students are required to present appropriate proof that the student received a health examination and the immunizations against, and screenings for, preventable communicable diseases within one year prior to:

1. Enrolling in an Illinois school for the first time, regardless of the student’s grade

2. Entering Kindergarten

3. Entering the sixth and ninth grades

*The required health examination must include a diabetes screening (diabetes testing is not required) and a statement from a physician assuring “risk-assessed”.

Failure to comply with the above requirements by October 15 of the current school year will result in the student’s exclusion from school until the required health forms are presented to the school, subject to certain exceptions. New students who register mid-term shall have 30 days following registration to comply with the health examination and immunization regulations. If a student has an exemption which is listed below, the documents required for the exemption must be filed with the school office by October 15 of the current school year.


A student will be exempted from the above requirements for:

1. Religious or medical grounds if the student’s parent/guardian presents to the building principal a signed statement explaining the objection;

2. Health examination or immunization requirements on medical grounds if a physician provides written verification.

Vision and Hearing Screening

Each fall and spring the Morgan County Health Department conducts vision and hearing screenings. If your child fails either of these screenings, you will be notified by the Health Department. Please be sure to follow up on recommendations.
